Xinlin here again.
to post the rd form for certain people who doesn't know how to keep their forms in their files immediately after they received it. (if you forgot to bring your entire file back then I have nothing to say)
Normally won't scan these things.
But I forgot off scanner and today I very happy so I scan xD
Erm the thing not so clear I know.
If not clear enough or for some other reasons, get someone else who can scan clearer (idk who. idk how, maybe got better scanner or editted or smth)
The back page of the form right, some lines are erased off or smth.
Cause at first I filled up the form alr.
Then I taped it.
Btw, I somehow believe that you're augustine (cause just now augustine got on msn. lols unreasonable reason;D )
But I have no access to the IP address so I'm not sure.
